ABSTRACT

Three dimensional (3D) bioprinting is a new biological tissue engineering technology in recent years. The development of 3D bioprinting is conducive to solving the current problems of clinical tissue and organ repairing. This article provides a review about the clinical and research status of 3D bioprinting and urinary system reconstruction. Furthermore, the feasibility and clinical value of 3D bioprinting in urinary system reconstruction will be also discussed.

ABSTRACT

Coping style is a cognitive or behavioral strategy taken by individuals in the face of stress. Positive coping style is of great significance for improving the physical and mental outcomes of elderly patients with urinary incontinence. Accurate assessment of coping styles for the elderly patients with urinary incontinence can provide reference for the subsequent development of intervention measures. The existing coping style assessment tools for elderly incontinence at home and abroad include specific scale of incontinence, relevant psychological assessment scale, and universal scale. In a word, the progress in the studies on relevant assessment tools is slow, and it mainly focuses on the assessment of female population. The assessment content is relatively single and lacks of pertinence and systematization. In the future, a comprehensive scale with strong adaptability should be developed based on the characteristics of elderly incontinence patients in China.

ABSTRACT

From April 2017 to September 2019, the START (standardized reception, teaching, analysis, research, training) method was adopted to train 50 general practitioners at the community general practice teaching clinic of Shanghai Huangdu Community Health Service Center. After 3 months of training, the progress of clinical competence and teaching ability were assessed. After training, the scores of clinical skills and communication skills were significantly higher than those before training (P<0.01). The scores of medical history taking, clinical thinking and health education were higher than before (all P<0.01). But there was no significant difference in record writing and reception time management between those before and after training (all P>0.05). The scores of the ability for building physician-patient relationship, collecting information and providing feedback, understanding complaints from patients′ perspective, sharing information and obtaining informed consents were higher than that before training (all P<0.01). The teaching ability of plan design and teaching method were significantly improved compared with that before training (all P<0.01). The satisfaction rate of START method was 96.0%(48/50). The START method applied in community teaching clinic can significantly improve the clinical competency and teaching ability of general practitioners.

ABSTRACT

The regional education platform is important in the training and continuing education for general practitioners, which provides clinical training program to upgrade the competence of general practitioner, but it is still in the initial stage. This article introduces the construction and development of inservice training and education platform for general practitioners in Shanghai Jiading district, and introduces the specific function and main features of the platform.

ABSTRACT

During kidney transplant, the non-specific inflammatory response induced by ischemia-reperfusion injury (IRI) will lead to decreased survival ability of transplanted kidney. However, the effect of IRI on long-term survival rate of allograft is not sure. Here we illuminated the relationship between early IRI and decreased long-term survival ability of allograft by retrospectively analyzing the clinical evidences and laboratory investigations. Previous studies showed that early IRI resulted in the graft loss through reduction of renal functional mass, vascular injury, chronic hypoxia and subsequent fibrosis. IRI was also one of the main factors to induce dysfunction of transplanted kidney and acute rejection reaction, and to decrease the allograft survival. Therefore, it's better to substitute traditional methods with novel measures during kidney transplant which may relieve the renal IRI much better.

ABSTRACT

Objective To investigate the significance of treadmill exercise test in male patients with coronary heart disease. Methods Two hundred and twenty-four male patients who diagnosed as coronary heart disease by coronary angiography were enrolled in this study, and treadmill exercise test were performed before and 1 month after coronary angiography. The coronary artery stenosis degree and scope were compared between two methods. Results In coronary angiography of 224 patients, the positive was 171 patients and negative was 53 patients. The single stenosis, double stenosis and triple stenosis was 87,55,29 patients. In treadmill exercise test of 224 patients, the positive was 143 patients and negative was 81 patients. The sensibility of treadmill exercise test compared the standard of coronary angiography was 81.3%,positive predictive value was 97.2%.Meanwhile in patients combined with hypertension or diabetic, treadmill exercise test found that the ST segment depression appeared earlier, persistence time longer. The persistence time and the number of down lead in double stenosis and triple stenosis was higher than that in single stenosis. Conclusion Treadmill exercise test is an important method in the diagnosis of coronary artery disease, and can preliminarily estimate the extent of coronary artery stenosis.

ABSTRACT

Objective: To investigate the effect of puncture-related pain on the quality of life in patients undergoing maintenance hemodialysis through internal arteriovenous if stula. Methods: A total of 180 hemodialysis patients with the arteriovenous ifstula were surveyed by the kidney disease quality of life short form(KDQOL-SF1.3), demographic data questionnaire, visual analogue scale and pain self-effcacy questionnaire. Results: The median score of puncture-related pain was 5 and the score of pain self-efficacy was (31.42±14.59). The quality of life in the patients undergoing maintenance hemodialysis is poor. KDQOL-SF1.3 was (69.45±24.19), SF-36 was (49.82±19.17) and ESRD-targeted was (55.46±18.37). Multivariate analysis demonstrated that the quality of life was positively correlated with the patient gender (β=0.152,P<0.05, OR=1.638, 95% CI 1.241–1.954), working position (β=0.307,P<0.05, OR=2.069, 95% CI 1.206–-3.148), using time of arteriovenous ifstula (β=?0.815, P<0.05, OR=0.223, 95% CI 0.095–0.741), the score of pain (β=-0.017,P<0.05, OR=1.004, 95% CI 0.886–1.431) and pain self-effcacy (β=-0.409,P<0.05, OR=0.803, 95% CI 0.710–0.984). hTere existed negative correlation between the quality of life score and the puncture-related pain score in these patients (r=-0.472,-0.465,-0.381,P<0.01), positive correlation between the quality of life score and the score of pain self-efficacy (r=0.647, 0.203, 0.518,P<0.05), and negative correlation between the puncture-related pain score and the score of pain self-effcacy(r=-0.745,P<0.01). Conclusion: Puncture-related pain is a crucial inlfuential factor on the quality of life in the patients undergoing maintenance hemodialysis through internal arteriovenous ifstula.
